Cost

Due to the complexity of care and time required to review each individual request, we charge a fee of $500.00 for this service. Payment is due at the time of the request and all major credit cards are accepted (Visa, MasterCard, American Express and Discover.) We do not accept payment by check.

Insurance

We do not submit claims or bills to insurance for this image review service. If you anticipate a trip to Vail for treatment or surgery and would like to learn more about your benefits and coverage please call your insurance company directly and provide them with the clinic’s TAX ID #84-1415470

To request a film review, please follow the steps below.

Step 1

  • To begin the review process download our image review packet HERE.
  • Complete the information contained in the packet.

Step 2

We do ask for all the below associated imaging reports for any and all imaging sent (within 1 year):

  • X-rays, MRIs/ CTs
  • physician clinic or treatment notes
  • operative reports with photos
  • any applicable clinical documentation for Dr. Provencher’s review

Please send copies of your imaging studies and not original films as they will not be returned (sorry, no exceptions).

Step 3

Send your images and records to our office and to the attention of Dr. Provencher’s Team. Delivery options are listed below:

Step 4

After all materials are received, a member of the team will contact you to confirm your image review is received successfully and will charge your card for payment in anticipation of the review. Lastly, Maddy, Dr. Provencher’s practice manager will reach out to schedule this image review phone call.

Step 5

Maddy will begin the image review phone call with a general medical history. She will call the phone number you provide in the packet. Dr.Provencher will then review your image findings and his treatment recommendations with you.

Please allow 7-14 business days for image review processing and phone call follow-up.
